incident reporting and investigation
The purpose of identifying, reporting, investigating and monitoring allegations of abuse and neglect, significant incidents and serious and minor notable occurrences is to ensure that people with special needs are protected from any level of harm or adversity and that they consistently receive the highest quality of care. The leadership team at General Human Outreach in the Community, Inc. (GHO) will adhere to this purpose by maintaining the highest standards and expectations for the health, safety and dignity of all individuals receiving services at GHO and ensuring that the dedicated men and women who provide those services are well trained and supported at all times. The primary function of reporting events and situations is to enable the governing body, management and supervisory staff to be aware and take corrective action, and minimize the potential for recurrence of similar situations. Prompt reporting ensures that any immediate steps are taken to protect people from being exposed to similar risks. This policy outlines the process whereby situations that endanger a person’s well-being while under GHO’s care are reported, investigated, and reviewed. Protective, corrective, and remedial actions will be determined as necessary based on reports, investigation and review.
