Family Support Assistant (FSS)

The FSS Assistant assists the individuals and their families to access necessary services through individual outreach and case management efforts and ensures that those services are being delivered in compliance with NYS/OPWDD regulations and contracted units of service.

Reports to FSS Coordinator and finally to the Deputy Director

Duties & Responsibilities:

· Assist the Family Support Services (FSS) Department with individual outreach, intake, and program/service enrollment processes; maintain the agency’s individual waiting list.

· Provide case management services to individuals in their homes.

· Obtain, verify, and maintain all required family documentation.

· Represent the agency at ID/DD Council meetings and participate in outreach events as needed.

· Coordinate all agency outreach activities and events.

· Provide service coordination for individuals enrolled in Family Support Services.

· Document activities including eligibility, enrollment, and monthly case notes in a timely manner.

· Support maintenance of quarterly unit service commitments and related reporting.

· Assist with Family Reimbursement responsibilities.

· Collect and organize all necessary materials, forms, and receipts for Committee review.

· Review receipts to ensure purchased items comply with reimbursement requirements.

· Prepare check request forms and submit for supervisor approval.

· Ensure compliance with the OPWDD contract by distributing required annual funds to eligible families.

· Participate in required staff training, supervision sessions, and staff meetings.

· Perform other duties as assigned.

Qualifications:

· Associate’s or bachelor’s degree in health or human service field preferred.

· At least 1-3 years of experience working in the Human Services field, preferably In the OPWDD field.

· Willing to train the right candidate with minimal experience.

· Bilingual Chinese required. (Cantonese and/or Mandarin)

IRA Manager

Dynamic professional needed to manage the day to day operations of a Queens residential home for individuals with special needs.

Duties & Responsibilities:

  • Maintain regulatory compliance with all federal, state and local regulations, policies, practices and standards applicable to the IRA program.
  • Comply with and implement Agency policies, procedures and practices governing programs.
  • Uphold and protect the rights of all individuals served.
  • Provide direct care services as necessary to the individuals who reside in the IRA(s).
  • Schedule staff and assign staff tasks as needed.
  • Supervise, monitor and evaluate staff performance and provide training as needed.
  • Work flexible shifts as assigned and comply with 24-hour on call requirement.

IRA Shift Supervisor

The IRA Shift Supervisor supervises all programs and activities on a given shift and ensures that all work assignments are carried out and that individual protection is maintained.

ACCOUNTABILITY

The IRA Shift Supervisor reports directly to the IRA Manager and finally to the Assistant Director of Residential Services .

DUTIES AND RESPONSIBILITIES OF THE JOB

1. As a Direct Support Professional, the IRA Shift Supervisor assumes all the duties and responsibilities of that job classification, including providing coverage as needed.

2. Provide supervision and assistance in individual programming and for all activities and work assignments on a given shift. Ensure that the proper documentation has been completed.

3. Handle emergency situations as the supervisor on duty in the absence of the IRA Manager.

4. Assist in the training of new Direct Support Professionals.

5. In the absence of the IRA Manager, schedule and supervise staff and relief workers, including AMAP coverage.

6. Assume managerial oversight responsibilities for the IRA program and ensure quality of care for individuals in the absence of the IRA Manager.

7. Have a working knowledge of federal, state and local regulations and policies and procedures applicable to the Agency’s IRA program.

8. Uphold and protect the rights of all individuals served.

9. Have a working knowledge of all individual’s goals to provide quality input at 6-month and annual review meetings.

10. In coordination with the IRA Manager, assist in purchasing food and household items.

11. In coordination with the IRA Manager, assist in purchasing individuals’ clothing and personal articles and verifying that the proper items have been purchased and are in the consumers’ possession.

12. In coordination with the IRA Manager, schedule medical appointments and attend as needed.

13. In coordination with the IRA Manager, plan recreational activities.

14. Attend staff meetings, supervision sessions and staff training as scheduled.

15. Provide crisis intervention as needed (upon completion of an approved SCIP course).

16. Provide First Aid and CPR when needed (upon completion of an approved First Aid and CPR course).

17. In coordination with the IRA Manager, review the Medex for accuracy and conformity to all OPWDD and Agency requirements and monitor the medication supply.

18. Assist in maintaining a safe and clean physical plant.

19. Perform other duties as assigned.

EDUCATION

High School Diploma required. Some college education preferred.

EXPERIENCE

2-5 years working with the ID/ DD population.

Some supervisory experience is preferred.

Clinical Director (LCSW or licensed Clinical Psychologist)-OPWDD Services

The clinical Director is responsible for the oversight and leadership of all clinical services within the programs regulated by the New York State Office for People with Developmental Disabilities. This role ensures that services are person-centered, trauma-informed, and compliant with OPWDD regulations, while promoting the health, safety, and well-being of individuals with developmental disabilities.

Qualifications:

· Must be a licensed Clinical Social Worker (LCSW) or a licensed Clinical. Psychologist (PhD/PsyD) in New York State.

· Active, unrestricted license required.

· Minimum of 5-7 years of clinical experience, preferably in OPWDD or similar settings.

· At least 2-3 years of supervisory or leadership experience.

· Strong Knowledge of OPWDD regulations, including Part 624/625, HCBS Waiver, and person-centered planning.

Key Responsibilities:

· Provide clinical oversight across residential, day habilitation, and/ or community-based programs.

· Supervise clinicians and ensure appropriate clinical interventions and supports.

· Oversee development and review of:

o Life Plans

o Behavioral Support Plans (BSPs)

o Functional Behavior Assessments (FBAs)

· Ensure compliance with OPWDD standards, including documentation, incident reporting, and audit readiness.

· Participate in Incident Review committees and ensure proper follow-up and mitigation strategies.

· Collaborate with interdisciplinary teams, including nursing, psychiatry, and direct support staff.

· Provide crisis intervention guidance and clinical consultation.

· Ensure implementation of trauma-informed and person-centered practices.

· Lead staff training on behavioral support, mental health, and clinical best practices.

· Monitor program outcomes and support quality improvement initiatives.

Skills & Competencies:

· Strong understanding of intellectual and developmental disabilities (I/DD).

Behavioral Intervention Specialist

The purpose of the BIS is to oversee the psychological and behavioral needs of the individuals in our IRAs and Day Hab programs.

ACCOUNTABILITY

The BIS reports directly to the Director of Clinical Services or designee.

DUTIES AND RESPONSIBILITES

1. Participate as a team member of the individual program planning.

2. Develop and monitor individual behavior management plans.

3. Maintain ongoing clinical notes.

4. Provide annual and semiannual behavioral information to the Care Managers for Life plans and six-month updates.

5. Provide training to program staff and participate in certified required trainings

6. Provide individual and group counseling to individuals as indicated in program plans.

7. Provide behavioral information to psychiatrists as needed.

8. Provide testing as needed.

9. Provide Psychological assessments for individuals in process applying for GHO services.

10. Participate in internal Agency committees. (i.e., IRC, HRC and Grand Rounds)

11. Ensure the health and safety of every individual.

12. Other duties as assigned.

EDUCATION

Master’s Degree in Psychology required.

EXPERIENCE AND TRAINING

At least 1-3 year experience working with the intellectually disabled population.
